We are currently seeking a passionate and experienced Clinical Case Manager Behavioral Health-Spanish Speaking to join our team. We are seeking individuals who are not only fluent in Spanish but also have a strong background in behavioral health and a deep understanding of the complexities of mental health conditions. Conduct comprehensive assessments of members' behavioral health needs and develop individualized case management plans. Provide ongoing support and advocacy for Spanish-speaking members with mental health conditions, ensuring access to appropriate healthcare services. Collaborate with healthcare providers and community resources to coordinate care and ensure continuity of services for members. Monitor and track progress of members, identifying any barriers or challenges to treatment and implementing interventions as needed. Educate members and their families on available resources and support services, promoting self-management and empowerment. Maintain accurate and timely documentation of all case management activities and progress. Adhere to all ethical and legal standards of practice, maintaining confidentiality and upholding the well-being of members. Stay up-to-date on current trends and best practices in behavioral health and utilize this knowledge to inform and improve case management services. Provide cultural competency and sensitivity in working with Spanish-speaking members, ensuring equitable and inclusive care. Collaborate with other members of the healthcare team and participate in interdisciplinary meetings to ensure holistic care for members. Identify and report any potential quality of care issues and make recommendations for improvement. Act as a liaison between members, providers, and insurance companies to facilitate efficient and effective care. Participate in ongoing training and development opportunities to enhance skills and knowledge in behavioral health and case management. Adhere to all company policies, procedures, and standards to maintain a safe and positive work environment.
